Job Description

JOB SUMMARY

 

Provides County-wide information, assistance, and access to Aging Services and community rsources for older adult refugees. Assesses, implements, coordinates, follows up, and evaluates options and services to meet clients' needs.

 

MINIMUM QUALIFICATIONS

 

Bachelor's degree from an accredited college or university in a Human Services field, Social Work, Sociology, Psychology, or other closely related field; OR an equivalent combination of related education and experience.

 

Due to the nature of this position, the successful candidate must pass a required pre-employment background check.

 

ESSENTIAL FUNCTIONS

  • Provides information and assistance to older adult refugees seeking access to services. Consults with clients and families regarding service eligibility requirements.
  • Responds daily to telephone, on-site visits, and walk-in inquiries and concerns relating to health and well-being including emergency or potentially life-threatening situations.
  • Makes referrals as appropriate for comprehensive assessments such as overall health, environment, and formal and informal support status, and annual reassessments for eligibility for homebound older adults seeking Meals on Wheels or other services.
  • Advocates for older adult refugees needing assistance by linking them to services.
  • Provides follow-up to assist older adult refugees to meet identified goals in their plan of care.
  • Coordinates appropriately to ensure customer safety including contacts with Adult Protective Services, Law Enforcement, Fire Department, Mobile Crisis Outreach Team, and other agencies as needed.
  • Develops and maintains working relationships with community partners who provide services to older adult refugees.
  • Attends community activities and provides information and education related to services for older adult refugees.
  • Maintains a waiting list for services for potential clients following established procedures.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS

 

The successful candidate will connect older adult refugees and families to services and resources that help them explore options, assist with independent living, and provide information about health care benefits and fraud prevention. 

 

TEMPORARY POSITIONS:

 

An "At Will", FLSA non-exempt employee who is not eligible for County benefits and who must work an average of 29 hours or less per week or an average of 129 hours or less per month.

  • Must work an average of 29 hours or less per week or an average of 129 hours or less per month.
  • Are not eligible for County benefits except as otherwise provided for by policy.
  • Are not considered merit employees.
  • Are "at will" employees who may be terminated for any reason, without notice, and without a pre-termination hearing.
