JOB SUMMARY
Provides social work case management to adults in home and community-based settings according to County, State, and Federal regulations, policies, and procedures. Provides information, awareness, marketing, and educational programs for caregivers and community.
MINIMUM QUALIFICATIONS
Bachelor’s degree from an accredited college or university in Social Work, Gerontology, or other closely related field; OR an equivalent combination of related education and experience.
Must be licensed as a Social Service Worker in the State of Utah.
Due to the nature of this position, the successful applicant must pass a required pre-employment background check and subsequent mandatory background checks in accordance with current County Human Resources policy requirements.
ESSENTIAL FUNCTIONS
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
Case Managers are often subjected to unsanitary conditions, pets, and unsafe neighborhoods.
Must be able to access transportation to perform work assignments in a timely manner.
