JOB SUMMARY
Provides nursing and case management services to assigned clients. Provides medical-legal record management, community outreach, education, and positive public relations.
MINIMUM QUALIFICATIONS
Professional licensure in the State of Utah as a Registered Nurse or a Multistate Nursing Compact License.
Due to grant funding Bachelor's degree is required.
Preference will be giving to applicants who are bilingual in English and Spanish.
Due to the nature of this position, the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.
A valid Driver’s License accepted by the State of Utah is required at the time of hire. A valid Driver’s License issued by the State of Utah must be obtained within six (6) months of hire to operate a County vehicle.
It is mandatory all new hires receive the Tdap (Tetanus, Diphtheria & Pertussis) vaccine before beginning employment or provide a copy of their immunizations card prior to starting. The immunizations card must show the vaccine name and date received.
Upon hire, a two-step Tuberculosis skin test will be required. The first will need to be placed prior to starting employment.

IMPORTANT INFORMATION REGARDING THIS POSITION
Time-limited Appointed employees will perform work for a specified amount of time dependent on agency funding. They are exempted by law from the rights of a merit employee and work as an at will employee.
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
This position works within the Nurse Family Partnership program It serves first time prenatal moms in the community. Services continue until the child is 2 years old. Services are provided in home, conducting clinical screenings and assessments to promote healthy childbirth, mothers, and children. This positon does require travel throughout Salt Lake Coutny and a flexible schedule to meet the needs of families served. This position generally works Monday through Friday, 8:00 am to 5:00 pm but may include some evening and weekend hours.
