JOB SUMMARY


Provides nursing and case management services to assigned clients. Provides medical-legal record management, community outreach, education, and positive public relations. This nurse works in the immunization group providing vaccines, education, etc. to clients in the 5 immunization clinics, and various outreach clinics around Salt Lake County, include travel clinic.


MINIMUM QUALIFICATIONS


Professional licensure in the State of Utah as a Registered Nurse or a Multistate Nursing Compact License.


A valid Driver’s License, insurance, and access to a private vehicle is required.

 

Preference will be given to applicants who are Bilingual in English and Spanish, and requires reading, writing, and speaking both English and Spanish fluently.

 

Due to the nature of this position, the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.

 

A valid Driver’s License accepted by the State of Utah is required at the time of hire. A valid Driver’s License issued by the State of Utah must be obtained within six (6) months of hire to operate a County vehicle.


Due to the nature of this role, vaccination or proof of vaccination may be required.

 

ESSENTIAL FUNCTIONS

  • Provides professional nursing services and case management to assigned clients in a clinical, community, and/or infectious disease/epidemiological setting.
  • Utilizes established standards of practice, nursing knowledge, and agency policies and procedures.
  • Assesses health needs and priorities using appropriate assessment tools.
  • Educates and refers clients to appropriate resources.
  • Maintains and facilitates a current referral/resource network and patient follow-up and monitoring system.
  • Provides timely interventions and prioritization of all work assignments and accurate and professional medical-legal documentation of client interventions.
  • Ensures compliance to all regulations pertaining to record management. (i.e., HIPAA and GRAMA).
  • Monitors assigned work area’s medical inventory, equipment or durable/ sellable goods, as may be assigned.
  • Prepares, analyzes and submits professional plans, guidelines, protocols, and reports.

  • Public Health Nursing services/interventions (i.e., immunizations, communicable disease control, epidemiology, home visiting)
  • Nursing scope of practice and limitations
  • Legal documentation and professional writing
  • Effective teaching practices
  • Cultural competency
  • Knowledge of community resources

  • Interview, counsel, educate, and conduct health assessments
  • Use computer software related to job specific duties including electronic medical records

  • Work cooperatively with clients, coworkers, professionals and others
  • Communicate effectively both orally and in writing
  • Organize and prioritize assignments and responsibilities and keep time expectations 

Additional Information

IMPORTANT INFORMATION REGARDING THIS POSITION

 

Regular Clinic hours are M-F from 8:00AM-5:00PM, Wednesdays are extended hours between 8:00AM-6:30PM. Able to rotate to different clinics, including Travel Clinic, across the Salt Lake County. Subject to temporary change based on community and clinic needs. Outreach clinic hours are varied but can range from Monday -Sunday 6:00AM to 8:00PM.

 

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS

 

• May be required to work in various environments with uncomfortable conditions, i.e. hot, cold, aromatic, or dirty.

• Must have the ability to stand, bend, sit for extended periods of time, or perform repetitive motor tasks.

• May need to lift up to 50 lbs. • Able to have transportation available to travel to events.

• May be required to be respiratory fit tested.

• Must adhere to OSHA blood-borne pathogens and respiratory precaution standards.
