JOB SUMMARY
Works with community members, Community Based Organizations (CBOs), healthcare providers, schools, religious organizations, other organizations within the community, and departments/programs within the County to strengthen health equity. Serves as a liaison between community members and service providers, addresses social determinants of health, and provides education in culturally relevant and linguistically appropriate ways.
MINIMUM QUALIFICATIONS
Bachelor’s degree from an accredited college or university in Community Education, Public Health, or in a related field of study, OR an equivalent combination of related education and experience.
Must possess a Health Literacy Certification within one (1) year of hire.
Community Health Worker Certification is strongly preferred. Experience working in specific zip codes/neighborhoods and with underrepresented communities strongly preferred.
Ability to read, write, and speak in a language other than English strongly preferred.
A valid Driver’s License accepted by the State of Utah is required at the time of hire. A valid Driver’s License issued by the State of Utah must be obtained within six (6) months of hire, to operate a county vehicle.
Due to the nature of this position, the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.
ESSENTIAL FUNCTIONS
- Conducts case management functions to determine eligibility, complete intakes, conduct screenings and assessments, and maintain accurate documentation and records following HIPAA guidelines
- Develops partnerships, provides connections and resources, and acts as a representative of the Agency by planning and conducting outreach to community agencies, groups, and coalitions.
- Develops and provides culturally and linguistically appropriate information, education, presentations, reports, trainings to internal programs, community agencies and groups about health department and county services, programs, and health topics.
- Collaborates with internal programs and community partners to discuss identified community needs, develop strategies, and coordinate services across agencies.
- Provides advocacy for clients and works closely with service providers to facilitate resource connection.
- Utilizes lived experiences and data to guide community leaders to address and evaluate population-based health issues.
- Assesses community health education needs and priorities using focus groups, interviews, and other forms of data collection and analysis.
- Facilitates and participates with community-based organizations as a Health Department liaison to identify health needs of a given community; helps prioritize and set goals for the community and provides direction, program expertise, and support of community-based health initiatives.
- Informs the development of culturally and linguistically appropriate health messaging, public service announcements and credible press releases based on the Culturally and Linguistically Appropriate Service (CLAS) standards at an internal and external organizational level.
- Empowers communities to advocate for themselves based on lived experiences.
- Performs community health-related special projects as needed.
- Develops and provides culturally and linguistically appropriate information, education, presentations, reports, trainings to internal programs, community agencies and groups about health department and county services, programs, and health topics.

IMPORTANT INFORMATION REGARDING THIS POSITION
Time-limited Appointed employees will perform work for a specified amount of time dependent on agency funding. They are exempted by law from the rights of a merit employee and work as an at will employee.
This position works Monday through Friday; May be required to work holidays, nights, weekends, or special events as needed. May be required to work non-typical hours.
